The first three forms are for new clients to fill out and bring to their initial therapy sessions. The fourth form gives me permission to contact your other clinicians and care providers. If you have any questions about these forms, please do not hesitate to contact me.
If you’re planning on attending one of my groups, please fill out the Group Confidentiality Agreement & Contract in addition to the above Consent Form and Office Policies Form and either email them back to me or bring them to the first group meeting.
